Senior Farmer’s Market Nutrition Program 2018
Provides $20.00 in free produce vouchers toUnion-Snyder County Seniors age 60 and Older
with a gross household income at or below
$22,459 for a single individual, and $30,451 for a 2 person household
Proof of age and residency is required (Driver’s license, photo ID, etc.).
You may only receive the vouchers once per year.

Produce vouchers will only be distributed at the above listed locations, dates, & times.
For eligible seniors who are unable to come to the distribution sites,
a designated representative may obtain a proxy form at the Union-Snyder Agency on Aging.
The form must be completed and signed by the eligible senior.
Proof of age and residence of the senior is also required to be presented with the proxy form.
Proxy forms are available at the Senior Centers and the Agency on Aging.
